1. Introduction

Martial arts training at Primal Combat Academy Limited involves physical activity and self-defence techniques that inherently carry a risk of injury. This form ensures that all participants (or their parents/guardians if under 18) understand and acknowledge the risks involved before engaging in training.

2. Acknowledgement of Risk

By signing this form, I acknowledge and understand that:

• Participation in martial arts involves inherent risks, including but not limited to bruises, sprains, strains, fractures, concussions, dislocations, and other potential injuries.
• More severe injuries, while rare, can occur, including head injuries, nerve damage, or serious trauma.
• The nature of martial arts training involves close contact with instructors, training partners, and equipment, which increases the risk of accidental injury.
• There is a possibility of experiencing emotional or psychological stress during training.
• There is a risk of exposure to communicable diseases (such as colds, flu, or COVID-19), particularly in a training environment where physical contact is involved.

3. Voluntary Participation

I confirm that:

• My participation (or my child's participation if signing as a parent/guardian) is voluntary and I am aware of the risks involved.
• I have had the opportunity to ask questions regarding the nature and risks of training at Primal Combat Academy Limited.
• I understand that I (or my child) have the right to stop participation at any time if uncomfortable with any aspect of training.

4. Safety and Responsibility

I acknowledge that:

• The instructors and staff at Primal Combat Academy Limited will take all reasonable precautions to ensure safety, but they cannot eliminate all risks associated with martial arts training.
• I (or my child) must follow all instructions, safety rules, and guidelines given by the instructors.
• I am responsible for ensuring that I (or my child) wear appropriate protective equipment when required and that I report any injuries or medical concerns to the instructor immediately.

5. Medical Conditions

I confirm that:

• I (or my child) am physically fit to participate in martial arts and have no known medical conditions that would pose a risk to myself, instructors, or others.
• If I (or my child) have any pre-existing medical conditions (such as asthma, epilepsy, heart conditions, or prior injuries), I have disclosed them to the instructor before training.
• I understand that it is my responsibility to consult with a medical professional if I have any concerns about my (or my child’s) ability to participate.

6. Waiver and Release of Liability

By signing this form, I agree to the following:

• I release and hold harmless Primal Combat Academy Limited, its instructors, staff, volunteers, and any associated parties from any and all liability for personal injury, accidents, or damages that may arise from participation in training.
• I understand that this release applies to any injury or loss that may occur due to negligence (except where prohibited by law) and includes injuries arising from the use of facilities, training methods, or the actions of instructors and training partners.
• This waiver does not affect my legal rights in the event of proven gross negligence or intentional misconduct by the club.

7. Emergency Contact and Consent

If signing on behalf of a child, I confirm that:

• I am the legal parent/guardian of the named participant.
• I give consent for my child to participate in martial arts training at Primal Combat Academy Limited.
• In case of emergency, I give permission for the instructors to administer basic first aid or seek medical assistance as necessary.

8. Confirmation and Agreement

I confirm that:

• I have read and understood this Assumption of Risk Form.
• I am signing this agreement voluntarily and with full knowledge of its contents.
• I agree to abide by the safety rules, policies, and procedures of Primal Combat Academy Limited.

9. Signature

To be completed by the participant (or parent/guardian if the participant is under 18).

This form must be signed before participating in any training activities at Primal Combat Academy Limited.

Primal Combat Academy Limited Terms & Conditions

1. Introduction

Welcome to Primal Combat Academy Limited. These Terms & Conditions govern your membership, training, and participation in any activities associated with the club, including in-person and online services.

By enrolling in our classes, using our website, or engaging with our club in any way, you agree to abide by these terms.

2. Membership & Fees

2.1 Membership Registration

All students must complete a registration form before participating in training. This includes providing relevant medical history, emergency contact details, and any additional safeguarding information where applicable.

By taking out a membership, you understand that you must purchase a club uniform in the first 4 weeks.

2.2 Membership Fees & Payments

• All training fees, membership payments, and associated costs must be paid in advance.
• Payment methods accepted include direct debit or other club-approved payment platforms.
• Failure to pay membership fees may result in suspension from classes until outstanding payments are settled.
• In the event of a failed or returned direct debit payment, a late payment fee will be applied.
• Membership fees are non-transferable.

2.3 Refunds

• Refunds will not be issued for missed sessions, unless in exceptional circumstances at the discretion of Primal Combat Academy Limited.
• If a student cancels their membership, a 30-day notice is required, and no refunds will be issued.
• Refunds for long-term injuries or illnesses may be considered with medical evidence.

2.4 Membership Cancellations

• All membership cancellations require a 30-day notice, unless in exceptional circumstances at the discretion of Primal Combat Academy Limited.
• To cancel a membership, you are required to email admin@primalcombatacademy.co.uk with your 30-day cancellation notice.

3. Class & Private Lesson Cancellations

3.1 Regular Class Cancellations

• Classes may occasionally be cancelled or rescheduled due to unforeseen circumstances, such as instructor illness or venue unavailability.

3.2 Private Lesson Cancellation Policy

• Private lessons must be cancelled at least 24 hours in advance to receive a reschedule opportunity.
• Failure to provide adequate notice will result in the full lesson fee being charged.

4. Code of Conduct & Behaviour

To ensure a safe and respectful training environment, all students and parents must adhere to the following:

• Respect instructors, fellow students, and all club personnel at all times.
• No foul language, bullying, or aggressive behaviour will be tolerated.
• Personal hygiene is a must to keep yourself and others from injuries or infections.
• Students must arrive on time, in appropriate training attire, and with the necessary equipment.
• Parents/guardians must respect instructor-led classes and avoid interfering with coaching unless invited to participate.

5. Safeguarding & Welfare

Primal Combat Academy Limited is committed to safeguarding children, young people, and vulnerable adults.

• All instructors and relevant staff are DBS-checked and receive safeguarding training.
• Any safeguarding concerns should be reported immediately to the designated safeguarding officer or to BMABA’s Safeguarding Team at safeguarding@bmaba.org.uk or by calling 01798 306546.

6. Use of the Club’s Website & Digital Resources

By accessing our website and online resources, you agree to:

• Use our website in accordance with UK laws and regulations.
• Not distribute, copy, or misuse any content from Primal Combat Academy Limited without permission.
• Not engage in any unauthorised sharing or distribution of club-provided materials.

7. Photography & Videography

From time to time, photographs and videos may be taken during training sessions for club marketing and promotional purposes.

• Any participant who does not wish to be photographed or recorded must notify the club in writing.
• Images and videos will be stored securely in line with GDPR regulations.

8. Liability & Assumption of Risk

Martial arts training involves physical contact and carries an inherent risk of injury.

• All students train at their own risk and must understand this is a full-contact sport and risks are involved.
• Primal Combat Academy Limited will not be held liable for personal injury, loss, or damage unless caused by proven negligence.
• Students are responsible for ensuring they are physically fit to participate and must disclose any medical conditions that may impact their training.

9. Data Protection & Privacy

Primal Combat Academy Limited complies with the UK GDPR and Data Protection Act 2018.

• Personal data collected from students is used solely for membership, safety, and communication purposes.
• Data will not be shared with third parties without consent unless required by law.

10. Complaints Procedure

If you have a complaint, please follow the procedure below:

• Raise concerns informally with the instructor or club manager.
• If unresolved, submit a formal complaint in writing to Primal Combat Academy Limited via email or post.
• If the complaint relates to safeguarding or is not resolved, it can be escalated to BMABA’s Safeguarding Team via safeguarding@bmaba.org.uk or by calling 01798 306546.

11. Amendments & Updates

Primal Combat Academy Limited reserves the right to update these Terms & Conditions as necessary. Any significant changes will be communicated via email or posted on our website.

12. Acceptance of Terms

By training with Primal Combat Academy Limited, you acknowledge and agree to these Terms & Conditions.

Primal Combat Academy Limited Photography & Videography Consent Form

Introduction

At Primal Combat Academy Limited, we occasionally take photographs and record videos of our students during training sessions, competitions, and events. These images and videos may be used for promotional and educational purposes, including but not limited to:

• Club websites and social media platforms
• Printed marketing materials such as flyers and brochures
• Training and educational resources
• Local or national media (with prior notice)

To comply with UK Data Protection Laws, including the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018, we require your explicit consent before capturing and using any images or videos in which you (or your child) appear.

Data Protection & Your Rights

Your personal data (including images and videos) will be processed securely in compliance with UK GDPR. You have the right to:

• Withdraw your consent at any time. To do so, please contact admin@primalcombatacademy.co.uk.
• Request the removal of any images or videos in which you or your child appear.
